Our Health Management Programs
To improve the quality, safety, and value of your health care, we offer programs to help you manage your care and get healthier.
Case Management
For members with chronic or complex care needs, we offer added support through our Blue Care Connection case management programs. Program members work with our registered nurses to improve their health and make decisions about their care. You may be identified to participate in one of our programs through a doctor's referral, a self-referral, or your claims data.
